Authorization for Payment and Membership Terms

By signing this agreement, I authorize 10th Planet Jiu Jitsu Club LLC to automatically withdraw payments from the credit card or bank account identified below for all charges associated with my membership. The information I have provided, including the details of my payment method, is true, current, and complete. I agree to promptly update this information as necessary to ensure its accuracy.

Automatic Payment Withdrawals

Payments will be automatically withdrawn from the designated credit card or bank account on the membership start date, and will continue until a written notice of cancellation is provided to 10th Planet Jiu Jitsu Club LLC. A minimum of 7 business days' notice before the next scheduled payment is required to revoke the authorization.

Freezing Accounts Policy

• Memberships can be frozen for a maximum of 10 days, twice per year.
• Services are available at the discretion of the member.
• Freezing memberships are subject to approval from the Academies management.
• Unused services will not be eligible for credits or refunds.
• Services are available for the duration of the membership period. If classes are not attended during this period, they cannot be rescheduled or carried over to a later time.

Membership Payments

All membership payments must now be processed through our website. Any payments made in person at the reception, whether by cash or POS machine, will incur an additional 5% administrative fee.

Online Registration

All members are required to register and manage their membership and training schedules via our website to ensure smooth operations.

Prepaid Memberships

Prepaid memberships are non-refundable.

Pregnancy

In the event of pregnancy, memberships must be frozen for safety reasons

Underage Students

All students under the age of 18 must be accompanied by an adult during all classes. The legal guardian or parent must sign this agreement on behalf of the minor.

Death or Permanent Disability

If the participant is unable to complete the program due to death or permanent disability, they will be relieved of any future payment obligations, except for services rendered prior to death or disability onset.

By signing this agreement, I acknowledge that I understand and agree to the terms outlined above.
